What's reasonably well established
Vitamin D deficiency is consistently associated with worse cognitive outcomes and higher dementia risk in observational studies. Vitamin D also appears to play a role in reducing inflammation and supporting neuron survival in laboratory research.
Where the evidence gets murkier
Clinical trials testing whether vitamin D supplementation actually improves cognitive outcomes in people who aren't deficient have generally not shown clear benefit — suggesting the association may partly reflect that vitamin D deficiency often overlaps with other risk factors, like reduced outdoor activity.
What this means practically
- Correcting an actual vitamin D deficiency, confirmed by bloodwork, is reasonable and may support overall health, including brain health
- Supplementing beyond a normal level, without a confirmed deficiency, has not shown clear additional cognitive benefit in trials
- Getting vitamin D checked by a doctor is more useful than assuming supplementation alone will meaningfully change cognitive trajectory
